How-To Report Damages/Issues with DMC Equipment and Spaces
Follow the steps below to report damages or issues with the equipment that is loaned out through Reftab, or if there are damages or issues in the TV Studio/Control Room (#120), or the Post Production Room (#122).
1. Open https://endicott.freshservice.com/support/home and select the red "New Ticket" button in the top right corner.
2. On the next page, fill in all required fields. For the subject, please provide what issue you're having or what type of damage you would like to report.
3. In the "Incident Type" field, select "Hardware Issue."
4. In the "Item" field, select "Digital Media Equipment Damage."
5. If you're reporting damages or issues with equipment loaned out through Reftab, please include in the description of the ticket what the Asset ID number is (this can be found on all gear loaned out by the DMC). Also, please describe in as much detail what damages there are or what issues you're experiencing with the equipment.
If you're reporting damages or issues to one of the learning spaces, please include in the description of the ticket what the room number is: #120 for the TV Studio/Control Room, or #122 for the Post Production Room. Also, please describe in as much detail what damages there are or what issues you're experiencing within the space.
6. Once all required fields have been filled out, you can select the green "Submit" button at the bottom of the page. A support representative will then review your report and will send you a personal response (usually within 24 hours).
